Greenfield, IN. This is a full-time Physical Therapist in the new
Employer Based Clinic setting.What makes this setting so unique?The
Employer Based Clinic setting is unique because it is not your
typical outpatient setting. You are the sole Physical Therapist and
have opportunities to collaborate with other healthcare providers
in the clinic. Patients and providers build relationships with you,
and you become a trusted provider.Dynamic patient caseload consists
of post-op orthopedic (general and total joints), back and neck
(manual therapy), other musculoskeletal conditions, and work comp
in a hands-on environment. This position only treats employees and
their dependents on a covered employer health plan.

requirements:
- Degree from a CAPTE accredited school
- Current state licensure in Physical Therapy in the state of
Indiana
- Has minimum one (1) year of outpatient experience
- Has minimum one (1) year of orthopedic and manual therapy
experience
- Experience with Work Injury Management and/or Work Comp a
plus
- Proficiency with EMR scheduling and documentation systems
- Confidence, initiative, and experience to build employer
relationships and patient caseloads
- Will display strong skills in flexibility, time management,
leadership, and communication
- Focuses on the patient and providing the utmost in customer
service excellenceThe responsibilities:
- Deliver quality, outcomes-driven physical therapy services
- Collaborate with other healthcare providers within the onsite
clinic
- Document with therapist friendly web-based software and
documentation system (EMR)
- Utilize FOTO patient engagement and outcome-based tools
- Manage relationships and promote physical therapy services
- Conduct patient and family education
- Display strong skills in flexibility, time management, and
