Registered Nurse (RN) | Case Manager Location: Indianapolis, IN Agency: Cynet Health Pay: $2,331 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/16/2026 About the Position Job Title: RN Case Manager Profession: RN Specialty: Case Management Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum 2 years recent RN Case Management experience, with experience in a long-term acute care or short-term acute care setting, integrated discharge planning across acute care settings, and strong interdisciplinary team collaboration License: Must have Indiana RN or multi-state RN license Certifications: Must have BLS certification Must-Have: - Comprehensive assessment and care planning skills - Expertise in care coordination and plan implementation - Strong patient advocacy skills Description: Coordinates management of care and ensures optimum utilization of resources. Ensures compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction. Promotes continuity of care and cost effectiveness. Integrates functions of case management, utilization review, and discharge planning. Addresses the holistic needs of the patient. Serves as a patient advocate through resource utilization and discharge planning. Ensures patient preferences are taken into account.
